Prescription Weight Loss Services Agreement

Please read these Terms of Service carefully before using our prescription weight loss services. By using our services, you agree to be bound by these terms.
1. Services Description
Glow's prescription weight loss services ("Services") include telehealth consultations with licensed healthcare providers, prescription medications for weight management, and related support services. These Services are provided in partnership with licensed pharmacies and healthcare providers.
Endo Health, Inc. ("Glow," "we," "us") facilitates these Services but does not directly provide medical care. Medical services are provided by independently licensed healthcare providers.
In connection with these Services, certain health information you provide may be processed by Endo Health's technology platform to facilitate your care coordination. For details on how your health information is handled, please see our Prescription Privacy Policy.
2. Eligibility Requirements
To use our prescription Services, you must:
- Be at least 18 years of age
- Be a resident of a state where we provide Services
- Provide accurate and complete health information
- Have a valid form of identification
- Have a valid email address and phone number
- Not have any medical conditions that contraindicate the use of prescribed medications
3. Medical Disclaimers
Not a Substitute for Medical Advice
Our Services are not intended to replace the relationship between you and your primary care physician or other healthcare providers. Always consult with your healthcare provider before starting, stopping, or changing any treatment or medication.
No Guarantee of Prescription: Completing a health assessment does not guarantee that you will receive a prescription. All prescribing decisions are made by licensed healthcare providers based on their clinical judgment.
Results May Vary: Weight loss results vary by individual. We make no guarantees regarding the amount of weight you may lose or the time frame in which you may lose it.
4. Compounded Medications
FDA Notice for Compounded Medications
Compounded medications are prepared by licensed compounding pharmacies but are NOT FDA-approved and have not been evaluated by the FDA for safety, efficacy, or quality. By accepting compounded medications, you acknowledge and accept these risks.
5. Pricing and Payment
Pricing for our Services includes:
- Telehealth consultation fees
- Medication costs
- Shipping and handling
All prices are displayed before purchase. We do not accept insurance for our Services. Payment is due at the time of order. We accept major credit cards and other payment methods as displayed at checkout.
6. Subscription and Auto-Renewal
If you enroll in a subscription plan, your subscription will automatically renew at the end of each billing period unless you cancel before the renewal date. You may cancel your subscription at any time through your account settings or by contacting our support team.
7. Refund Policy
Consultation Fee: If your consultation does not occur or your prescription is not approved, you may be eligible for a refund of the consultation fee. Contact support@glowdiet.com for consultation fee refund requests.
Medication Costs: Medication purchases are processed directly by our partner pharmacies. For medication-related refunds, returns, or questions, please contact the dispensing pharmacy directly:

Due to the nature of prescription medications, pharmacies generally cannot accept returns once medications have been shipped.
8. Telehealth Consent
By using our Services, you consent to receive healthcare services via telehealth technology. You understand that:
- Telehealth involves electronic communication of medical information
- Technical failures may occur and interrupt services
- Telehealth may not be appropriate for all medical situations
- You have the right to refuse telehealth services
- Your health information shared during telehealth consultations is protected under HIPAA
- You consent to the electronic transmission of your health information between healthcare providers, pharmacies, and the Glow platform as necessary to provide your care
9. HIPAA Acknowledgment and Health Information Consent
By using our prescription Services, you acknowledge and agree to the following:
- Your health information may be shared with our healthcare partners for the purposes of treatment, payment, and healthcare operations
- Endo Health, Inc. acts as a technology platform facilitator and does not directly provide medical care. Clinical Protected Health Information (PHI) is primarily collected and managed by our healthcare partners
- Certain health-related information you voluntarily share through the Glow platform (such as weight data, medication adherence logs, and wellness coaching context) may be processed by our technology systems to facilitate your care
- Endo Health maintains Business Associate Agreements (BAAs) with its technology partners to ensure HIPAA-compliant handling of health information
You acknowledge that you have received and reviewed our Prescription Privacy Policy, which describes in detail how your health information is handled, your rights under HIPAA, and our breach notification procedures.
10. Your Responsibilities
You agree to:
- Provide accurate, complete, and current health information
- Notify us of any changes to your health status
- Use medications only as prescribed
- Not share your prescription medications with others
- Seek immediate medical attention if you experience serious side effects
- Comply with all applicable laws regarding prescription medications
11. Health Information and Data Practices
In connection with our Services, the Glow platform may process the following health-related information that you voluntarily provide:
- Weight and body measurements
- Medication adherence logs
- AI wellness coaching conversation content
- Apple Health data you choose to integrate
- Wellness goals and dietary preferences
Our services are intended for maintaining and encouraging a healthy lifestyle and are not used for the diagnosis, cure, management, prevention, or treatment of any disease or condition. Our services should never be used for medication management or dosing decisions. All medical decisions, including prescribing, are made solely by licensed healthcare providers.
In the event of a breach of unsecured health information, we will notify affected individuals without unreasonable delay and no later than 60 calendar days from discovery, in accordance with the HIPAA Breach Notification Rule.
12. Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, ENDO HEALTH, INC. AND ITS AFFILIATES SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF OUR SERVICES. OUR TOTAL LIABILITY SHALL NOT EXCEED THE AMOUNT YOU PAID FOR THE SERVICES IN THE 12 MONTHS PRECEDING THE CLAIM.
13. Governing Law
These Terms shall be governed by and construed in accordance with the laws of the State of Delaware, without regard to its conflict of law provisions.
14. Changes to Terms
We may modify these Terms at any time. If we make material changes, we will notify you via email or through our Services. Your continued use of the Services after such notification constitutes acceptance of the modified Terms.
15. Severability
If any provision of these Terms is found to be unenforceable, the remaining provisions will remain in full force and effect.
